Overview
Long COVID is a chronic condition occurring after SARS-CoV-2 infection that may involve fatigue, cognitive symptoms, autonomic dysfunction, sleep disturbance, pain, respiratory symptoms, and other systems.
What current evidence indicates
- Long COVID is heterogeneous; NIH RECOVER trials are organized around symptom areas rather than assuming one mechanism or one treatment fits everyone.
- The RECOVER-VITAL randomized trial reported on August 31, 2026 found that extended 15- or 25-day nirmatrelvir/ritonavir did not improve overall Long COVID symptoms compared with placebo.
- RECOVER groups participants by common symptom patterns, including cognition, autonomic symptoms, and exercise intolerance. Additional NIH-supported trials are studying immune modulation, fatigue, autonomic symptoms, cognition, sleep, and other targets. A trial being underway does not establish that its treatment works.
What remains uncertain
There is no FDA-approved treatment specifically for Long COVID. Mechanisms, diagnostic biomarkers, subtypes, prognosis, and the effectiveness of many proposed treatments remain active research questions.
